So that we can provide convenient and accessible medical care to the community, we utilize place-based wellness hubs and mobile pop-up facilities at a number of public housing sites located in Washington, D.C. This allows us to reach the most under-resourced communities in the area. We also have plans to expand our services to additional locations.
Our clinics are overseen by a nurse practitioner and staffed by qualified, knowledgeable, and experienced healthcare staff – including an on-site public health nurse and community health worker. This is to ensure all patients get the high-quality medical care they need to improve and maintain their health.

How Does Telehealth Work?
At C3 Cares, we also offer telehealth visits to allow patients greater access to our health providers when they are unable to meet face-to-face or visit during daytime clinic hours. Telehealth uses existing technology, such as a smartphone, to allow patients to meet with a healthcare provider remotely, such as from the comfort of their own home or while at work.
Telehealth is an effective, quick, and convenient service where patients can discuss their health concerns or symptoms, or have follow-up care for existing health concerns. During a telehealth visit, medications can be prescribed, lab tests or other tests can be ordered, and if necessary, a face-to-face appointment can be scheduled.
